School Forms Fee Waiver - Middle School (one per family) Field Trips – Authorization Form Field Trips – Authorization to Use Privately Owned Vehicles on School Business Field Trips – Liability Release Student Withdraw Forms & Procedures If you are planning on withdrawing your students from Early Light Academy, you must complete the form below and return it to the front office PRIOR to leaving the school. If your student will be attending a Jordan School District school, you will also need a letter from the school your child will be attending stating that they have room for your child. If your student decides to leave Early Light Academy and wishes to return, they will need to go through the lottery process with other prospective students.
